NOT MEDICAL ADVICE: The laboratory testing services provided by Healthstyle Labs are for informational purposes only. The results do not constitute a medical diagnosis and are not a substitute for professional medical advice. Always seek the advice of your physician regarding any medical condition or treatment.

AGE REQUIREMENT & MINORS: If you are under the age of 18, you may only use this Service with the involvement and express consent of a parent or legal guardian. By placing an order for a minor, the parent or guardian acknowledges they are legally authorized to consent to medical testing for said minor and assumes full responsibility for the account and the interpretation of results with a qualified healthcare provider.

NO DOCTOR-PATIENT RELATIONSHIP: Use of this website or purchase of a test does not create a doctor-patient relationship between you and Healthstyle Labs.
2. Placing Orders & Testing Requirements
By placing an Order, you warrant that you are legally capable of entering into binding contracts.

Testing Eligibility: You agree that you are ordering these tests for your own health monitoring.

Lab Requisitions: Once an order is placed, we provide a lab requisition. This requisition is valid only for the person named on the order.

Accuracy of Information: You must provide a valid name, date of birth, and contact information. Providing false information may invalidate your test results.
3. Cancellation & Refund Policy

General Refunds: Due to the clinical nature of our services, refunds are generally only available if the lab requisition has not been used. Once a blood draw or specimen collection has occurred, no refunds will be issued.

Processing Fees: Canceled orders may be subject to a small administrative processing fee.

Non-Refundable Items: Tests that have expired (typically 3 months or 90 days after purchase) are not eligible for refunds.
4. Results & Mandatory Reporting

Confidentiality: We strive to maintain the highest level of privacy. However, you acknowledge that under certain state and federal laws, positive results for specific "reportable" diseases (e.g., HIV, Syphilis, Hepatitis) must be reported to the Department of Health by the performing laboratory.

Delivery of Results: Results are delivered via a secure electronic method. Healthstyle Labs is not responsible for delays caused by laboratory processing or technical issues outside of our control.
5. Limitation of Liability
To the maximum extent permitted by applicable law, the entire liability of Healthstyle Labs and its suppliers shall be limited to the amount actually paid by you through the Service. In no event shall the Company be liable for any special, incidental, or consequential damages (including, but not limited to, damages for personal injury or loss of privacy) arising out of the use or inability to use the Service.
6. "AS IS" Disclaimer
The Service is provided to you "AS IS" and "AS AVAILABLE" with all faults and defects. Healthstyle Labs provides no warranty that the Service will meet your requirements or achieve any intended results. We do not guarantee that the laboratory results will be 100% error-free, as clinical testing is subject to biological and technical variables.
7. Governing Law
The laws of the State of Arizona, United States, shall govern these Terms and your use of the Service. Any disputes arising from these terms shall be resolved informally first, and if necessary, in the courts located in Maricopa County, Arizona.
